Statement of Purpose

 

Northgate Village Surgery is a General Practice situated within the Cheshire & Merseyside Integrated Care Board (CMICB) and is open to all patients living within our practice boundary in Chester City Centre and surrounding areas. We are part of the Chester Central Primary Care Network (PCN) and work within local and national governance, guidance, and regulations to provide high-quality primary care services.

As a training practice, we are committed to the continuing education and training of doctors, practice staff and other healthcare professionals.

Our vision is to deliver the best quality of medical care as possible to our patients by providing effective, safe, and patient-centred care while maintaining dignity and respect as well as recognising equality and diversity.

Our mission is to achieve our vision by creating a collaborative and rewarding experience for both patients and members of the practice team, that allows views and needs to be heard and actioned upon appropriately. To ensure that all staff employed by Northgate Village are trained and competent to assist in the smooth and safe running of the practice to the benefit of all users.

·         To provide a high standard, safe and professional Primary Care Service to all our patients

·         To be committed to our patient’s needs – working collaboratively with the patient, their family, and carers to ensure their experience is positive, maintaining their independence and assuring they are involved in decision-making surrounding their treatment and care.

·         To provide our patients and staff with a ‘safe space’, offering support and protecting them from abuse

·         To focus on prevention of disease by promoting health and wellbeing, and

·         To work in partnership with our Primary Care Network and other agencies to tackle the causes of, as well as provide the treatment for ill health, and where appropriate involve other professionals in the care of our patients.

Essential

We provide essential services for people who have health conditions from which they are expected to recover, chronic disease management and general management of terminally ill patients. Our core service include:

·         Asthma Clinics

·         Chronic Obstructive Airways Disease (COPD) Clinics

·         Coronary Heart Disease Clinics

·         Diabetes Clinics

Team of Patient Services Administrators

 

Our Patient Services Administrators manage reception duties at Northgate Village Surgery. They answer calls, emails, and patient queries at the front reception desk; arranging various appointments, producing patient repeat prescriptions, give results, signpost to appropriate services, and explain our services. Our administrators have various extended roles and responsibilities i.e., Carer Lead, Registrations and Deductions and Summarising, and are required to manage and communicate all information regarding this.

Our Secretary supports our GP’s and other healthcare professionals in all administration of referrals and communication between secondary care and they surgery. The secretary can answer patient queries concerning these communications. Our secretary also arranges all non-NHS services and acts as an intermediary between the patient and the GP.

Our Medicines Manager oversees all tasks and duties related to patient medications. This involves activities such as actioning and work flowing changes on discharge summaries/clinic letters, undertaking medication monitoring to ensure safe prescribing, answering patient/admin team queries, and undertaking Prescribing Scheme Work set by CMICB.

Our Data Quality Manager processes all correspondence received in the practice, this involves the workflow and coding of letters. She undertakes regular audits of our achievements and operates an effective recall system for patient reviews; this is carried out via month of birth. Another aspect of this work is overseeing the patient registration process, including GP2GP record transfers, deductions, amendments, and transaction queries.
